Trinity Community at Fairborn

I placed my father here in hopes I would get to spend time with him before his passing. It was closer to home, and I heard management had changed and things were better. When he was transferred here not one single person tried to contact me regarding his admissions. How their facility works, who his medical team would be, have me sign any intake paperwork or show them my poas. I had to take my poas in myself. I requested to meet with admissions and asked about intake forms or welcome packet. They did not meet with me but instead the front desk simply said she would email me. She indeed did not. The 2nd day of him being there a nurse yelled at me for asking the nurse to get him in a wheel chair. Stated that he could only be moved by a machine. Keep in mind he was not using one at the last facility also no medical team had relayed this message to me. After about 2 weeks they moved someone in with him no communication there. Nurse staff seemed to be in shock I had no idea, even gave me suggestions on facilities I could move him to. After meeting with the only manager that seemed to care and venting my frustrations then I had a meeting with the social worker and some other lady. Now lets fast forward to the worst part. I come in and my father was able to communicate, eat, joke around. 3 days later hes bed bound and his wrist is hurt. I received no call stating he was injured. The nurses all seemed clueless. Luckily I had already placed him in hospice. I called my hospice nurse in (which this is their job) she spoke with trintys Dr and they agreed it was either sprained or he had gout. Why was i the one thay caught it. Two days later come back and hes in more pain. Same thing nurses dont know, hes worse, I once again call my hospice nurse in. She comes out and orders comfort meds as she believes he is starting his transition. Next day I come back hospice comes back. Trinity has yet to give my father his pain meds for comfort. Stated they didnt even know he was in hospice. I immediately move him that day and thank god i did. Instead of enjoying what little time I had left with my father I was busy catching things fairborn of trinty gets paid to do. Ive called their administer, guess who hasn't called back? They withheld his medications, he was injured and no one knows what happened, he declined and no one reported it to his poa. I dont even blame the nurses. If you are that short staff dont take in people if they cannot be taken care of. If you cant meet with the family to help the transition get better management.

My brother-in-law has been a resident at Trinity for almost three years and I am impressed with the level of care, friendliness of staff, and responsiveness of all administrators. The very few problems encountered were discussed with staff and solved. My brother-in-law feels safe and loved, which enables him to express his wants and needs comfortably and without worrying about how they are received. I would recommend Trinity to anyone needing care.

Just no! Food limp and low quality for sick people. Had to take food over several times.They give meds together for continuance but interaction risk. I don't recommend even for short stay. way to many pain meds and pain from their therapy after 35 days rehab. I do not recommend. Addiction to drugs like narcotics is risk. Protect your loved ones. Some places better choices.

The staff is kind and patient regardless of the circumstance. I appreciate their caring attitude.

Excellent staff, including clinical, administrative and maintenance personnel, who do their best to care for patients with varying degrees of physical and cognitive abilities. Our 98 year old mother has been a AL resident at Trinity Community Fairborn, OH for 3 plus years, following an illness requiring treatment with blood thinner. Staff members are patient and kind with her dementia- which is sometimes challenging . Activity center staff works hard to engage residents, while respecting individual residents’ personal wishes. This is largely a Christian community - several residents have connection to prior military service. The food is typical of similar institutions - room for improvement.
