Big changes have been made since my original rating of this facility. They have made improvements upon the safety of my mother and other residents. I would recommend this care home.

I cannot speak highly enough about Hazelwood Enhanced Memory Care. From the moment we first visited, we felt a true sense of warmth, compassion, and professionalism from the entire team. The staff consistently goes above and beyond to ensure residents are treated with dignity, respect, and personalized care. The facility is always clean, safe, and thoughtfully designed for individuals living with memory challenges. What truly stands out is their commitment to meaningful, personalized activities. I’ve personally witnessed how well-maintained their activity programming is—Ms. Donna, the Life Enrichment Director, starts her mornings engaging the residents with meditation, simple exercises, and other thoughtful activities that truly brighten their day. These small but consistent efforts make a big difference in the lives of the residents.

I visit the facility almost every day, and I’ve seen firsthand how every member of the team genuinely cares. Communication with families is excellent—whether it’s regular updates or addressing concerns, the administrator makes sure to keep me informed about my mom’s care, which gives me great peace of mind. We are incredibly grateful to have found Hazelwood Enhanced Memory Care – it’s more than just a care facility; it’s a community where residents are truly valued. Initially, my family felt Hazelwood would be a good fit for my mom, but over the past two months of her nearly six month stay, my opinion has declined sharply. While I believe my mother is safe, and her basic needs are being met, I have significant concerns about this facility. They have fallen short of everything they offered when we toured the facility; activities, 30-60-90 day formal reviews, cleanliness, staffing ratios, etc. There do not seem to be many structured activities - most of the residents are sitting idly or wandering. (Which I know can be normal, but I have had family on my wife's side in memory care and that was not the case.) There have been no formal reviews about my mother's care or situation. The staff is communicative regarding her status, but it's usually along the lines of "she is doing great!". The cleanliness of the facility has become a problem to me. There is a consistent urine odor and the general areas do not typically feel clean or maintained. There have been Super Bowl decorations that have not been removed for months, and Christmas decorations by the front door since we moved my mother in February 2025. The staff has constant turnover. I know it is not easy to find qualified care workers, but this place is poorly managed from the top-down. I do not blame the workers, but I cannot tell you how often I observe staff completely checked out and not engaged in the care of the residents. To me, that is a management problem, not a worker problem. Generally speaking, I would not recommend Hazelwood Advanced Memory Care to anyone seeking placement for their loved one. We are now considering other options. It is my sincere hope, for the sake of the residents, that this place gets its act together, but they need to make some significant changes and hire qualified managers to make that happen.
