Elmwood Assisted Living at The Shawhan is a 53-suite / 66-bed residential care facility in downtown Tiffin, Ohio, originally a historic 1904 hotel renovated and reopened as an assisted living community in 1999. The facility offers 24-hour professional nursing, assisted living, Alzheimer's and memory care services, with classic décor, elegant dining rooms, and a dedicated Reminiscence Neighborhood for residents with memory impairments.

Three-Room Suite
Most spacious suite with three rooms and a bathroom. May be used in any fashion to accommodate sleeping and living space preferences. Affectionately referred to as 'Penthouse Suite' with abundance of closet space and large windows.
Studio Suite
Available in a number of configurations with plenty of space devoted to living and sleeping areas. Many large windows, kitchenette, private bathroom with walk-in shower and generous closet space.
Private Suite
Sized appropriately for one person. Large windows, kitchenette, and private bathroom with walk-in shower make this ideal for an individual.

My mom resides there. It is absolutely beautiful and the staff are outstanding

This facility is pretty to look at but if your loved one has Alzheimer’s Disease or Dementia, I am concerned over lack of Activity programming on the 4th floor. There is great lack of communication between staff from the Nurse Aides to the nurses to administration. They rarely addressed my 8+ complaints over my Mom’s care….well really LACK of it. One nurse in particular was exceptionally rude and condescending. I had problems with Mom’s laundry, dietary issues, problems with getting her hair washed or showers done, and her being in bed so much of the day. Literally out of 14 out of 15 visits to her, I walked in to find her in bed all but one of those visits. I came to see her different hours on different days and found the same result. Lastly all administration dropped the ball on answering if their facility accepts Medicaid when your loved one runs out of money. ***Need to add on here. We moved her into another, far better facility, and their Nursing staff at Elmwood at the Shawhan or person in charge of sending info on my Mother, to the next facility, dropped the ball and only sent her medication list and financial info. There was no effort to share any info with them on my Mom’s ongoing stomach pain, GI Dr. notes, upcoming Endoscopy appt, Dr’s facility notes, past medication list of what she had been on while she was in their care, etc. I would NOT recommend this facility to anyone. They just kept forgetting to take care of important things and did not provide my Mom’s next facility with ANY continuity of care. I had to call the Ohio Dept of Health in multiple times and Ombudsman. The best thing I can do is give this review so other families can decide if they want this facility. We went down a road of unacceptable and negligent care, in our opinion.

This facility is beautiful. Always decorated for the holidays. The staff really cares about the residents. We have very little turn over which says wonders about the management at this facility.

My Mother is there and she loves it. She enjoys the activities and she says the food is good. She said she does not feel like she is in a nursing home. She was in a nursing home before we moved her there.

This is a must see at Christmas time, the staff goes all out for decorating!
