Welcoming, caring, friendly environment. Staff are knowledgeable and professional in all facets of care. Building is older, but clean as whistle, well maintained, and some updates are recenly completed.

My Mother was in the nursing home for rehab. EVERYONE was extremely nice and cared for her beyond measures. This is an amazing team. Thanks for ALL that you do on a daily basis. It does not go unseen.

I was a patient at Green Prairie in Plainview in February 2025, and I definitely give the facility a 5 star rating. The nursing care, OT and PT were exceptional. Everyone was so kind and caring. I received excellent care in all aspects. I would recommend this facility to anyone who needs some after care rehabilitation following hospital stays.
