Over the time my mother lived at Elk Creek Assisted Living & Memory Care, we experienced repeated concerns with her care, supervision, communication, and leadership. I was repeatedly assured that staff were being retrained and that improvements were being made, yet the same serious problems continued. The most heartbreaking part was witnessing caregivers who were impatient, dismissive, and at times cruel toward vulnerable residents. I personally witnessed staff taunting my mother, speaking harshly to her, and failing to provide the attentive care and dignity every resident deserves. I also found prescribed medications in her bed that she should have taken, repeatedly had to advocate for her basic care, and watched situations where residents were not being adequately supervised, resulting in a serious injury to my mother and an ER visit for 5 staples in her head. During another visit, I witnessed what I believed to be a very concerning response to a fire alarm and visible smoke in the building. No one was evacuated and staff did not seem to know what they were supposed to do. Instead of spending my visits simply enjoying time with my mom, I found myself constantly monitoring her care, documenting concerns, and advocating for her needs. Despite countless conversations, emails, phone calls, texts, letters, and meetings with leadership, lasting improvements never occurred. Because I could no longer trust that my mother would consistently receive safe, compassionate care, our family made the difficult decision to move her to another memory care facility. I sincerely hope Elk Creek makes meaningful changes. Families entrust them with the care of those who are unable to advocate for themselves, and that responsibility requires compassionate caregivers, strong leadership, adequate staffing, and consistent accountability.

⭐☆☆☆☆ Our family’s experience at Elk Creek Memory Care ended after a serious incident involving our mother that raised significant concerns about resident safety and supervision. The final straw occurred when another memory care resident entered my mother’s room and got into her bed. Our room video footage shows an escalating altercation that lasted for nearly an hour. During that time there was loud screaming, arguing, and obvious distress. Despite the duration of the incident, intervention did not occur before my mother was injured they came in later and found her on the floor. As a result of the altercation, my mother fell, struck her head, and was transported by ambulance to the emergency room, where she received five staples. The video also shows that after the ambulance left, the other resident involved was not immediately located. It shows on our video that the resident was later found in my mother’s bathroom. This raises serious concerns about resident monitoring and supervision within the memory care unit. This means this resident was MIA for over an hour and a half. Adding to our concerns, when my mother moved out, Elk Creek documented the reason as “transferred to a higher level of care.” That is a false statement That was not the reason for her departure and they are aware. We removed her because of our family’s dissatisfaction with the care, supervision, safety concerns, and handling of multiple incidents during her stay. She went to same level of care in different facility that so far has shown they actually do the care and follow the care plan. Families place loved ones with dementia in memory care because they need protection and supervision. In our experience, those expectations were not met. The existence of video footage documenting this incident makes these concerns especially troubling. I encourage families considering this facility to carefully investigate staffing levels, supervision practices, incident response procedures, and how resident safety concerns are handled before making a decision. And above all please have a camera in your lived ones room!! Mom has been at her new place for about a month. The wonderful change in her shows ! The care and attention, the way they keep her clean and interact with her and all the residents . Mom is now interacting, smiling, looks and acts like a completely different person than she was when we had to remove her from elk creek for their total lack of care or concern.

I would not recommend this facility to anyone looking for dementia or memory care for a loved one. Our family’s experience was deeply disappointing and heartbreaking. My grandmother suffered a quite serious injury while in their care. Beyond the injury itself, we were extremely concerned by what we observed regarding the overall quality of care, communication, supervision, and attention to residents’ needs. As a nurse practitioner myself, I have a solid understanding of the standard of care expected in long-term and dementia care settings. Based on what I personally witnessed, I believe this facility fell well below those expectations. Residents with dementia require vigilant supervision, compassion, and timely intervention, and our experience left us with significant concerns about whether those needs were consistently being met. Families place enormous trust in facilities like this during one of the most vulnerable times in their loved one’s life. Unfortunately, our experience left us feeling that trust was not honored. I encourage anyone considering this facility to ask detailed questions, visit frequently, and carefully evaluate whether it is the right place for their loved one.
