The director Lakeitha Webb and the social worker Shenesa has some kind of 'un natural' relationship which is jeopardizing the mental well being of its residents. If you cross the social worker, the director will make patient pay with constant mental harassment. My brother has been here for over 2 years with no problems and when the new director Lakeisha Webb and the social worker..'Shawanda'? Who constantly calls on Sunday multiple times because (Rodney who is a dialysis patient and suffers from severe arthritis and on medication for pain, is grouchy as most older nursing home patients are) she accused him of smoking and he along with many of the workers smoke in the outdoors break area, when told he couldn't smoke the mentioned that staff smokes. He agreed not to in a grouchy way,l. Has obliged, however the mental badgering continues. They called a bus without notifying the family to move him to a facility against his will in Hobart Indiana. I told the director he has complied and informed her of his rights of a 30 day notice. The social worker Shawanda ir Shaneeka called his dialysis in Munster center to let her know if he smoked at dialysis. I told her hes not on Dyer nursing property and if he wants to smoke outside away from everyone that's his rights. I asked the director who is running the place her or the social worker..she replied both of them are. Lakeisha is very much controlled..unnaturally by the social worker. They are very immature to run the facility. Im not a smoker, but those that do so respectfully have that right. These women should not be allowed to run this facility. When my brother first arrived it was a great facility with the dining room be filled, now most of the patients have died or left. If I could give this facility zero stars that’s what I’d rate it!!! This place is negligent, lacks customer courtesy and patient compassion!! The admissions person makes sure to sell the place as if it’s the best quality care your loved one will get. Dont fall for the bait! All other staff is unprofessional and they do not care about the patients & only there to collect a paycheck for doing nothing!!There has been several times where I’ve walked in and a patient or patients have asked the CNA’s for help and they walked past the patient and rolled their eyes and said I’ll be right with you… 3 hours later I leave , and the patient is still in the hallway asking for assistance, I took it upon myself to take the patient back to their room. Another time a patient is yelling for help because the CNA ignores the call light.., a CNA walks in the patients room and yells out “what do you want?”,And shuts the door and a few seconds later walks out.., patient is still asking for help.., my loved ones room was not cleaned properly. I’d clean it every other day with bleach & Mr clean. a few days after I removed my loved one from that hell hole, there was fecal excrement still on the floor, that was there from when my loved one had a movement on herself a week and half , prior to me removing her from that place!!! The place is not handicap accessible. My loved one fell out the bed and no one reported it to me. They had a CNa who’ would say that her name was “Thing” . She told my loved ones roommate that she didn’t want to give her name because she didn’t want them to report her, so she could get fired!!! The place smelled Like urine,, patients would sit outside of their room Doorway and plead for help.. CNA or front receptionist or other staff would not address you when walking by.. all you ever heard when passing the nursing huddle station was “I got my hair braided by” I ate this at so and so restaurant”, “ I wore this to the party” “my man this” , “my mom made this” “ I smoked” !! CNa’s always on their phones, iPads, or just sat there waiting for their shift to end., there was a time when I walked up to the “huddle station” and asked for something and no one looked up and asked if I may be helped. I asked for assistance again and no one budged. I waited there 10 minutes , just because, and listened to convos, and then walked back to the room and called the unit manager.., 30 minutes later loved one is assisted. Patients wait almost 2 hours to be changed.., they don’t take patients out of bed, no baths are given on their bath days (the excuse is I have to collect trays or you have to wait for your nurse. The furniture is broken and outdated!!! The administrator and the lady with all the snacks in her office have newly updated office furniture and they act like they care but in reality they don’t. Everyone in this negligent hell hole is only there to collect a check, while letting their staff mistreat the patients!!!

My experience with Cynthia Evans-Director of Nursing was not great. I found her communication to be rude, dismissive, and unprofessional & just nasty. I hope others have a better experience and that customer service improves.
